Total Entry Fees:

Teachers please complete this form (or one of your own that includes the same information) and enclose it with your registration, approval and certification forms. Please bring a copy of the completed form(s) to registration at Columbia Center on Thursday.

Instead of the students waiting in line, each teacher will be responsible for confirming entries and

paying any fees. The registration fee is $ 5.00 per student. MAKE CHECKS OR SCHOOL PURCHASE ORDERS PAYABLE TO MID-COLUMBIA REGIONAL SCIENCE FAIR ASSOCIATION.
